OVERVIEW:

CNA is responsible for providing direct resident care including clinical, personal, nutritional and restorative care.

RESPONSIBILITIES:

·         Help patients use the restroom

·         Clean and bathe patients or residents

·         Maintain patients' hygiene and change bedding

·         Turn, reposition, and move patients between wheelchairs, seats, and beds

·         Serve meals and help patients eat their food

·         Measure vital signs like blood pressure and temperature

·         Listen to patient’s health concerns and report details to a RN or Doctor

REQUIREMENTS:

·         Valid CNA License in the State of New York

·         CPR/BLS certified

·         Minimum 1 year nursing experience in a Long term care Facility.

OVERVIEW:

Under the leadership of the Registered Nurse, the LPN will be providing coordinated care to residents in a skilled nursing facility.

RESPONSIBILITIES:

·         Provide direct care responsibilities to patients

·         Supervise CNA’s  

·         Administer medications and treatments timely and according to policy

·         Receive, transcribe, and carry out physician orders

·         Follow established procedure for charting

·         Complete patient assessments, coordination of care

·         Identify and reports changes in condition

·         Follows environmental health & safety and sanitation standards and procedures

·         Oversees clinical and non-clinical acute operations and continuum of care

JOB DUTIES:

• Supervises the clinical care planning and delivery processes to ensure coordination of quality patient care. Reviews and updates the care plan accordingly
• Assesses residents/clients upon admission and periodically as conditions change
• Performs rounds to evaluate patient condition
• Provides assistance to deliver direct patient care, including activities of daily living, meal assistance, medication and treatment administration when indicated
• Obtains and implements Physician Orders, checking for accuracy and completeness
• Provides clinical assessment to MD/NPs on call for quality outcomes delivered to patients
• Implement and evaluates the clinical competencies and on-going educational/orientation needs of the staff.
• Participate in Facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services
• Ensuring sufficient number of nursing staff is available for each shift 
• Make daily rounds of the Nursing Services Department to ensure all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report all findings to the Director of Nursing Services
• Review complaints and grievances made or filed by department personnel. 
• Assist in the Quality Assurance and Performance Improvement (QAPI) process

QUALIFICATIONS:

• Current registration and license to practice as a Registered Nurse in the state employed in
• Peripheral IV certification
• Minimum 5+ years nursing experience in a hospital, nursing home or other healthcare setting and a minimum 1-year experience as a nurse supervisor
• CPR/BLS certified
• Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers.
• Bachelor of Science in Nursing
• The ability to read, write, speak and understand the English language required, knowledge of other language helpful

• File Fair Hearing and attend if necessary
• Identify budget discrepancies and correct when necessary
• Assist with enrollments/dis enrollments HMO/MLTC
• Collect private/NAMI payments as necessary
• Apply for Representative Payee as necessary
• Submit Social Security Annual report
• Guardianship petitions when necessary
• Other duties as needed to ensure department operates at its maximum

QUALIFICATIONS:

EDUCATION / EXPERIENCE:

• A Bachelor's degree in Human Services, Finance or related field
• 3 years of experience in Medicaid/Finance services
• Bilingual preferred but not necessary

·         Evaluates all residents/patients who have been referred by their physicians with a written order. Develops a treatment program including functional long and short term goals for all the residents/patients placed on a Speech therapy program.

·         Provides treatment as ordered by the attending physician. Interprets, plan, establishes, and exercises proper treatment program in accordance with the physician's referral, periodically, reviewing the resident's/patient's program and response and making recommendations to the physician relative to the need of continuing or discontinuing the Speech therapy treatment.

·         Contacts the physician for resident's/patient's reports and care as needed. Observes, records, and reports to the physician the resident's/patient's reactions to the therapy programs or any changes in the resident's/patient's condition.

·         Instructs, teaches and supervises residents/patients, and family/caregivers, regarding Speech therapy procedures as related to the resident's/patient's therapy programs.

·         Participates in staff development activities. Participates in in-service education programs.

·         Supervise CFYs as per the guidelines set forth by ASHA.

·         Adheres to all applicable federal, state, local, facility and company (BHI) maintained standards of care and ethics and policies concerned with the practice of Speech therapy and among other things, personnel qualifications.

·         Meets productivity and utilization expectation as established by the company.

·         Completes timely clinical documentation to include functional goal setting, progress and therapeutic approach.

QUALIFICATIONS:

·         Master’s degree in Speech-Language Pathology

·         Certificate of Clinical Competency or eligibility for Clinical Fellowship Year

·         Current state licensure

·         Provides Occupational Therapy services to patients under the supervision of an Occupational Therapist

·         Assists the Occupational Therapist in the evaluation process as needed or directed.

·         Identifies indications for and assists in evaluations at appropriate intervals under the direction of an Occupational Therapist.

·         Assists the Occupational Therapist in identifying a problem list, short term goals, long term goals and treatment plan.

·         Conducts individual, supervised and group treatment as assigned by the Occupational Therapist.

·         Monitors patient’s response to intervention and consults with Occupational Therapist if modification is needed.

·         Maintains acceptable patient care ratios as determined by the Occupational Therapist.

·         Provides/recommends/fabricates adaptive devices or other equipment; trains patient and caregiver in the use or application of orthotics and prosthetics.

·         Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient.

·         Assumes responsibility for ongoing continuing education and professional development.

·         Adheres to Occupational Therapy Department procedures regarding documentation of Occupational Therapy Services.

·         Provides treatment in accordance with established standards of practice, department procedures and productivity standards.

·         Records billable treatment times according to Occupational Therapy Department procedures and submits weekly/monthly statistics to regional office.

·         Complies with all Infection Control, Universal Precautions and OSHA standards for the healthcare professional.

·         Assists with and participates in Quality Assurance Programs as required.

·         Adheres to established confidentiality standards.

·         Remediating instrumental activities of daily living (IADLs) related to the patient’s discharge environment, such as preparing a meal or managing one’s home or finances

·         Training in functional mobility, such as how to prepare a meal while using an ambulatory device

·         Preparing the client and family for community reintegration (as appropriate for the client’s discharge site) with activities such as public dining or emergency response management

·         Assessing the need for and recommending potential home modifications and safety equipment to reduce barriers and promote safe functioning upon discharge

·         Exploring adaptations and compensatory strategies for return to volunteer or paid employment

·         Assessing current leisure skills to determine whether modifications are needed to continue participation and/or assisting with exploring new leisure pursuits

·         Teaching functional mobility, including using an ambulatory device and/or transfers to different surfaces, such as a bed, chair, toilet, or shower in order to perform self-care and personal tasks; and training in wheelchair mobility and safety appropriate for the resident’s level of cognition and perception

·         Remediating or enhancing IADLs, such as ability to use the telephone or the emergency staff call system

·         Teaching residents with cognitive and perceptual deficits in compensatory techniques to maximize abilities in areas such as attention span, orientation, sequencing, and/or memory

·         Teaching residents with low vision how to maximize their remaining vision and enhance safety through compensatory techniques, environmental modifications, assistive technology, and adaptive equipment

·         Instructing residents and caregivers in techniques to improve positioning, reduce the need for restraints, and maintain skin integrity

·         Modifying the environment and adapting tasks for safety (e.g., falls prevention, fatigue management)

·         Educating caregivers on ways to enhance function and preserve dignity by using the remaining abilities of adults with dementia

·         Providing opportunities for engagement in meaningful activities despite client limitations

QUALIFICATIONS:

EDUCATION / EXPERIENCE:

·         Master of Science in Occupational Therapy from an accredited program

·         Successful completion of National Certification Examination

·         1 year of verifiable, supervised professional experience within the last 3 years

Valid OT license in the state of practice

·         Reviewing patients’ medical histories

·         Diagnosing patients by observing their movements and listening to their concerns

·         Developing individualized treatment plans for patients

·         Outlining clear goals for patients and the expected outcomes of the plan

·         Using exercises, stretching, equipment, and hands-on therapy to manage patients’ pain, increase mobility, and prevent further pain and injury

·         Recording patients’ progress and modifying the plan of care if needed

·         Educating patients and family members about the recovery process

·         Consulting with patients to learn about their physical condition and symptoms

·         Assisting patients with the use of equipment such as wheelchairs or walkers

·         Advising the patient and family about in-home treatment options and exercises

·         Must be experienced at cleaning with a keen eye for details

·         Maintains a high level of cleanliness & safety throughout designated areas

·         Ensure that all equipment is clean and in working condition at all times

·         Removal of trash & soiled linens

·         Keeping the resident’s environment clean at all times

·         Cleaning, mopping and vacuuming floors, dusting furniture, emptying trash, clean and disinfect showers and bathrooms

·         Assist with laundry when needed

·         Reports equipment/cleaning product needs and or malfunctions to supervisor

·         Follows infection control and universal precautions policies and procedures to ensure that a sanitary environment is maintained at all times.

·         Follow proper reporting, isolation and handwashing procedures/techniques

·         Assist in setting up dining room, activity room, and living room as necessary

·         Keep all housekeeping equipment, storage areas, janitor’s closets, and housekeeping carts clean and uncluttered, and free of any hazardous equipment and supplies

·         Report all accidents/ incidents to the Administrator on the day of occurrence, along with important resident observations
